What do I need for a first-time adult appointment?

• If you are a first-time applicant or an applicant who does not qualify for a renewal passport, see the U.S. Department of State's requirements below.

U.S. Department of State Application Form(s)

A completed, but unsigned, DS-11 application from the U.S. Department of State. Some applications and circumstances may require additional forms and documentation. Uncompleted forms or missing documentation may require scheduling a new appointment. 

Forms can be filled out online and printed from the U.S. Department of State website. 

Proof of U.S. citizenship

• An original or certified copy of a U.S. Birth Certificate
• A U.S. Passport or certificate of naturalization
  • Note that the original copies may be sent to the U.S. Department of State with your application, per their requirements.

Proof of identity

• Current Driver’s License OR State ID card
  • A secondary form of ID or another approved form of ID is required if showing a non-Ohio driver’s license or an Ohio State ID card

One 2x2 passport photo

• Photos may be taken at Shaker Library facilities for $10.80 per applicant. Photos must match the U.S. Department of State's requirements.

A non-starter personal check, cashier’s check, or money order for the total amount required by the U.S. Department of State.

• Optional fees, such as a $60 expedite fee, are paid to the U.S. Department of State.
• Per U.S. Department of State requirements, an acceptance fee of $35 will be charged per application. This fee is paid to the library by cash or credit card only. 

What do I need for a minor appointment?

• Minor applicants are those ages 15 and under. Minor applicants cannot renew their passports. Minor must be present, including the parent(s) listed on the birth certificate, at the appointment.

• Visit the U.S. Department of State's website to confirm your eligibility for a passport renewal. Minors under 16 cannot apply for a passport renewal; they must apply in person using a DS-11.

• Photos may be taken at Shaker Library facilities for $10.80 per applicant. Photos must match the U.S. Department of State's requirements.

• There is no $35 acceptance fee for renewal appointments. 

Please note that our acceptance agents will review your application for completeness, but mailing the application is the responsibility of the applicant.

Scheduling an Appointment

Passport and photo services are available by appointment online or via phone at 216-991-2030, option #4. 

A one-hour appointment can accommodate up to 2 applications. You must book consecutive one-hour appointments for additional applications.

Passport Fees

There are two fees associated with passport services: 

• Fees to the Shaker Library
• Fees to the U.S. Department of State

Shaker Library Fees

• Shaker Library requires cash or credit/debit for payment types.
• A $35.00 acceptance fee per new adult or minor application, mandated by the U.S. Department of State. 
• $10.80 per applicant for a 2x2 passport photo.

U.S. Department of State Fees

• The U.S. Department of State requires its fees to be paid via personal check, money order, cashier's check, or traveler's check. Please see the table below for further information.
• The amount will vary depending on the type of passport selected and any additional options chosen, such as a $60 expedite fee.
